This category recognises a local or regional partnership that has made a significant impact on a national healthcare challenge — demonstrating the power of place-based collaboration and innovation.

Successful entries will show how a local approach can lead to tangible improvements in outcomes, service delivery, or efficiency, while offering clear potential for wider replication. Whether co-designed with patients, communities, or local systems, these initiatives exemplify how thoughtful partnership working at the local level can spark national change.

If your project is improving care locally while pointing the way for others across the country, we want to hear your story.
