The judges will be focusing on the following areas when scoring your entry. Write your entry accordingly.
- Executive Summary (200 words, not scored) This will be used if your entry is selected as a finalist. Please state clearly why this event is award-worthy – what is unique, differentiating and innovative about it.
- Depth of Transformation (600 words, 30 points) – How significantly the partnership has changed healthcare processes, policies, or outcomes.
- Evidence of Success (500 words, 25 points) – Measurable data showing the impact of the transformation.
- Sustainability & Integration (400 words, 20 points) – Plans for long-term adoption and integration into the healthcare system.
- Innovative Methods (300 words, 15 points) – How new models, technologies, or approaches have driven the transformation.
- Stakeholder & Patient Feedback (200 words, 10 points) – Testimonials or case studies showcasing impact.
